We each see the world differently, according to our personal and unique experiences in life. We seldom see things as they are, but will instead see things as we are, through the interpretation and meaning that our mind gives. This can make communication quite tricky - because you now have what actually happened, what one person perceived to have happened and what the other person perceived to have happened. It's up to you to let go of what you define as the 'facts' and take into consideration the reality for the person you are communicating with - see what they see, so that you may more effectively support them to consider other possible perspectives.
